The aim of the hub is to coordinate research, innovation and improvement activity in North Wales about how health and social care services can work together better. It is part of the Welsh Government commitment in A Healthier Wales to establish a nationally coordinated network of hubs to inform new integrated models of health and social care.

Learning from COVID-19 survey
We are collecting information about changes that have occurred as a consequence of the pandemic and we would like to know about the changes that have taken place in your workplace/organisation. There are a few ways to contribute:
- Submit details of the innovations and changes made through our online survey
- Share a story about the changes made with us, by arranging a phone call or video call.
- Email us with details of the innovation that took place, we’re happy to take information in any format, for example, case studies you’ve already written up.
This report includes the information received by 20 August 2020 and is being shared as a draft to check whether anything has been missed.
If you are aware of anything else that should be included, please let us know by the end of September 2020 so we can include it in the final report by emailing email@example.com.
