1. North Wales 10-year Strategic Capital Plan
The North Wales Regional Partnership Board, (referred to as ‘NWRPB’, ‘we’ or ‘us’ throughout this document), along with the other six Regional Partnership Boards in Wales, was tasked by the Welsh Government in 2022 with developing and implementing a Strategic Capital Plan for North Wales.
The Strategic Capital Plan reflects a 10-year view of the capital investment needs of North Wales to support the provision of health and social care services.

2. The people we support
The North Wales 10-year Strategic Capital Plan supports a range of demographics across the region. these include:
- children and young people
- older people
- people with physical disabilities
- people with learning disabilities and autism
- people with mental health problems
- people with sensory impairments
- carers needing support
- women suffering violence and domestic abuse

4. Completed schemes
Bwthyn y Ddol

Bwthyn y Ddol is a new assessment centre for children and families with complex needs, keeping children close to home, school, family, friends and support networks.
The centre enables health and social care professionals to work together, in one location, alongside children and families to improve their health and wellbeing.
Sylva Gardens

Sylva Gardens is a registered children’s home for 3 children with learning disabilities. The home comprises 3 apartments, each accommodating 1 young person and a sleep in room for a carer.
The home was completed in February 2025 and registered in May 2025. The first young person moved in on 20th May 2025.
The home provides care for children closer to home and is intended to be a home for life for all 3 young people should they wish to stay.
Ty Croes Atti

Tŷ Croes Atti is an older person’s residential care home with integrated health services in Flint, Flintshire.
The new home will be managed by Flintshire County Council as one of three in-house residential care homes and will provide a larger, modern replacement for the existing Croes Atti Care Home, also in Flint. The home is planned to be operational in the summer of 2025.
5. Schemes in construction
Coed y Ddraig

Coed y Ddraig is an integrated service hub in Mold, Flintshire.
Located in a rural woodland setting, only a short walk from Mold town centre, Coed y Ddraig will provide a purpose-built day and work opportunities service for individuals with learning disabilities, autism, and mental health support needs. The hub will be managed by Hft, with integrated service provision from social services and health board teams. The hub is planned to be operational in the summer of 2025.
Canolfan Glanhwfa

Canolfan Glanhwfa is being developed and run as an Integrated Community Hub in Llangefni, established to provide support services for older people in the community.
Phase one of the adaptation work is already complete and includes renovation to the vestry to provide a new kitchen, toilets and three offices. Phase two and phase three work is now underway and includes work to the lower floor, lift installation, and further restoration work to the chapel itself.
Dolfeurig Community Hub

A new community hub is being developed in Gwynedd to support people with learning disabilities.
The hub will provide new and innovative opportunities, including training and supporting people with learning disabilities to take their first steps into the world of work, encouraging collaboration and integration with the local community.
The first image depicts an artists impression of the completed hub. The second image below, shows the site currently as work as begun on the foundations and substructures.
Construction of the hub is planned to be complete in Spring 2026.
6. Funding
There are two main sources of Welsh Government Capital funding to support the Regional Partnership Board’s initiatives.
- Housing with Care Fund – supports the provision of supported housing and accommodation for vulnerable people with care and support needs.
- Integration and Rebalancing Capital Fund – supports the Programme for Government commitments of developing 50 integrated health and social care hubs and to support rebalancing the residential care market.
7. Housing with Care Fund
The Housing with Care Fund is a Welsh Government capital programme with the primary objective to increase the stock of housing to meet the needs of people with care and support needs. You can access the Housing with Care Fund guidance document here.
The Housing with Care Fund supports three overarching objectives:
- Supporting the ageing population
- Providing care closer to home
- Supporting intermediate health and care services in the community
North Wales have received £14.2 million of Housing with Care Funding for the first 3 years of the fund. In the 2025/26 financial year, North Wales allocation is £13.4 million
8. Integration and Rebalancing Capital Fund
The Integration and Rebalancing Capital Fund is an All Wales Welsh Government funding pot that every region of Wales can bid for. You can access the Integration and Rebalancing Capital Fund guidance document here.
It supports three overarching objectives:
- Supporting a coherent approach to planning the co-location and integration of health and social care services within the community across Wales. The delivery of Integrated Health and Social Care Hubs
- Supporting the rebalancing of adult residential care provision by increasing delivery from within the not-for-profit sector.
- Supporting the elimination of profit from the provision of children’s residential care.
The NWRPB work closely with partners to support a joined-up approach to the planning and delivery of health, social care and housing capital investment.
We want to enable the people of North Wales to access seamless service delivery closer to home.
Regional Partnership Boards have a key role in taking forward the development of integrated health and social care hubs as well as rebalancing the market under the Integration and Rebalancing Capital Fund guidance.
