The Welsh Government have made a new fund available to transform health and social services as set out in A Healthier Wales: our plan for health and social care.

We are pleased to announce that the North Wales Regional Partnership Board bid for a transformation programme has been successful. More information is available below and further information about how to get involved will be shared on our website.
