1. Who we are

All six Local Authorities and the Health Board work together with Alzheimer’s Society, Carers Trust North Wales, Carers Outreach and NEWCIS to make sure that the pathway is fair, easy to access, and person centred for everyone living in North Wales. 

Working together makes sure that people receive what matters to them. Over the phone, or face to face, we provide advice, guidance, support, and the opportunity to meet others who are living with dementia in a safe and friendly environment.

4. Worried about your memory? Have a diagnosis of dementia or caring for someone with dementia?

We aim to provide support, at the right time, to anyone living in North Wales who is affected by dementia or is worried about their memory, living with dementia at home or in hospital, or is an unpaid carer, relative or friend.

7. The North Wales Memory Support Pathway

On your journey through the Memory Support Pathway, you can access a range of services provided by third sector organisations, Alzheimer’s Society, NEWCIS, Carers Trust North Wales and Carers Outreach.

Appointments to discuss your memory concerns or those of your family member or friend can be arranged via your GP, who will first make sure you are physically well. If your memory or brain health concerns are not caused by physical illness, you will be referred to the Memory Assessment Team for a clinical assessment.

If you are concerned about your memory or those of your family member or friend and wish to access support or discuss the assessment process before visiting your GP, please contact your local Dementia Centre.

The Alzheimer’s Society provide pre assessment advice, support and signposting at the beginning of the Memory Support Pathway following your consent for a referral via the GP or the Memory Assessment Team. You can self-refer using the referral form at the top of the page or by contact us by phone or email.

The Memory Support Pathway Advisors are on hand to provide pre diagnosis support and information to anyone in North Wales who is worried about their memory or is an unpaid carer. The Memory Support Pathway Advisors will work with you to identify your individual needs, while empowering you to achieve what matters to you.

The Advisors will assist you on your journey through the Memory Support Pathway and your clinical assessment, making sure it is as straight-forward as possible by working closely with the Health Board Memory Assessment Services, Dementia Centre Coordinators and Hospital Navigators to provide:

  • 1:1 information, guidance, advice and support
  • Telephone support
  • Support sessions at the Dementia Centres
  • Drop-in sessions at the Dementia Centres

Clinical assessment and diagnosis is provided by Betsi Cadwaladr University Health Board’s Primary Care Services, including Older People’s Mental Health Services, Audiology and other associated clinical services.

The clinical assessment team includes:

  • Nurses
  • Psychiatrists
  • Occupational Therapists
  • Psychologists
  • Radiologists
  • Audiologists
  • Healthcare Assistants

The team also supports people after diagnosis with individual personalised care and providing interventions within people’s homes and community therapy sessions.

Regional Audiology Service (care for your ears)

If you or your family member or friend are referred for a clinical assessment, an appointment will be offered to check your hearing. We know that hearing and memory are linked together, and hearing loss can affect your memory. We can provide hearing aids for those undergoing a clinical assessment or living with dementia who experience hearing loss.

At the appointment an Audiologist will ask you or your family member or friend some questions about your or their hearing, check ears and perform a hearing test to see if any hearing loss is found.

Post diagnostic advice and support services are provided by Local Authorities, the Health Board, Alzheimer’s Society, Carers Trust, Carers Outreach and NEWCIS, in the community and in hospital.

If you or your unpaid carer receive support from the Memory Support Pathway Pre-Assessment Education and Support Services via Alzheimer’s Society, you will automatically be provided post diagnostic support. If this support has not been received, you can self-refer using the referral form at the top of the page or by contact us by phone or email.

These services are here to help you after you have had a clinical assessment and diagnosis of dementia. You will be supported by the Alzheimer’s Society, NEWCIS, Carers Trust and Carers Outreach. Dementia Advisors, Dementia Centre Coordinators and Hospital Navigators will work together to support you throughout your journey.

Our Dementia Centres have been established since 2022 and we have one in each County in North Wales. You can ask the dementia centres for support yourself or someone else can contact them for you.

Support following admission into hospital

Carers Outreach and NEWCIS provide a hospital navigator service at Ysbyty Gwynedd, Glan Clwyd Hospital and Wrexham Maelor Hospital.

If you are admitted into hospital and have a diagnosis of dementia or are experiencing memory related concerns, the Hospital Navigators will support you by:

  • Providing information
  • Signposting
  • Advising

The Hospital Navigators will support you for the duration of your stay in hospital until discharge and continue to support you as you progress onto the next steps of the Memory Support Pathway.

You can receive support from the Hospital Navigators if you have never accessed support before or if you are already receiving support from a provider on the Memory Support Pathway.

Once you have been discharged from hospital, the Hospital Navigators will refer you for pre/post diagnosis support, depending on your needs. If you are not seen by a Hospital Navigator during your stay in hospital, you can contact them using the referral form at the top of the page, by phone on 01492 542212, or email nwmemorysupport@ctnw.org.uk.
